Documents To Bring When Claiming Your LTO Plate Number
FOR REGISTERED OWNERS
- Original or Photocopy of OR/CR (Official Receipt/Certificate of Registration).
- Photocopy of one (1) Government issued ID with signature.
AUTHORIZED REPRESENTATIVE (IF OWNER IS NOT AVAILABLE)
- Original or Photocopy of OR/CR (Official Receipt/Certificate of Registration).
- Authorization Letter of the owner.
- Photocopy of one (1) Government issued ID of registered owner with three (3) specimen signatures.
- Government issued ID of the authorized representative with signature.
CORPORATE ACCOUNT
- Original or Photocopy of OR/CR (Official Receipt/Certificate of Registration).
- Authorization from the company’s authorized signatory.
- Photocopy of one (1) Government issued ID with three (3) specimen signatures of authorized signatory.
- Company ID or Government issued ID of authorized representative.
